Case Manager, QMHA (Residential Case Management - Float)

Job Summary
Provide mental health case management and skills training to clients in a community health center setting as part of the treatment team. Manage referrals, assess treatment needs, advocate for clients, and coordinate services including housing, benefits, and access to resources. Deliver CPST, activity therapy, and ensure alignment with trauma-informed care, harm reduction, Motivational Interviewing, and the Recovery model. Collaborate with outside providers, participate in staff meetings, and maintain compliant documentation while promoting client rights, safety, and professional boundaries. Requires a Bachelor’s degree (or equivalent experience), QMHA credentialing per MHACBO/OARs, a valid driver’s license, and BLS certification (provided if needed). The role operates across multiple sites in the Portland/Gresham area with in-person responsibilities Monday–Friday, 8:30 a.m.–5:00 p.m., and involves coordination with community partners and supervisors.
Required Qualifications
- Bachelor’s degree in a behavioral science field from an accredited college or university or three years of full-time professional experience in social services
- Credentialed as a Qualified Mental Health Associate (QMHA) per Oregon Administrative Rules (OARs) and MHACBO compliance
- Valid driver’s license with minimum one year driving history and ability to pass DMV check
- BLS certification (training provided within first 60 days)
