Job Summary
Manage a caseload of up to 20 individuals with developmental disabilities by developing individualized strategies, coordinating internal and external services, and ensuring compliance with DHS, HCBS, and CARF requirements. Complete monthly progress reviews, conduct unannounced and scheduled site visits, and provide direct feedback to staff on safety, rights, and confidentiality standards. Coordinate health, behavioral, and community services, including scheduling medical appointments and managing medication administration authorizations. Monitor client financial status, reconcile records, and assist with housing applications while maintaining accurate documentation in the electronic case record system. Perform physical assistance tasks such as transferring clients and providing transportation, and participate in direct service coverage as needed. Work Monday through Friday with flexibility for evening and weekend hours to meet client needs.
Required Qualifications
- Clinical case management experience
- Knowledge of DHS, HCBS, and CARF requirements
- Ability to coordinate internal and external services
- Experience with electronic record systems (Cx360)
- Understanding of client rights and privacy standards
- Ability to obtain and maintain required consents
- Knowledge of Social Security, Medicaid, and Public Aid guidelines
- Ability to conduct unannounced site visits
- Ability to conduct scheduled site visits
- Experience with financial oversight and benefit eligibility maintenance
- Ability to provide direct feedback and training to staff
- Experience with solutions-focused language and positive approaches
