Job Summary
Case Manager responsible for providing case management and care coordination for patients enrolled in addiction treatment. Develop, implement, manage, and evaluate patient care plans; convey clinical information between insurance providers and the treatment team as warranted; coordinate continuum of care activities to ensure optimum utilization of resources and adherence to external agency requirements. Responsibilities include coordinating patient assessments, treatment planning, crisis intervention, linking patients to community resources (financial housing, food, clothing, medical, educational, vocational), facilitating involvement with self-help groups, increasing social support networks, coordinating treatment and discharge activities, attending team meetings, maintaining confidentiality, and completing required trainings. Must meet education and competency requirements, maintain supervision, and be able to travel up to 15%.
Required Qualifications
- High school diploma or equivalent
- Training or education in mental health or substance use disorder competencies
- Ability to demonstrate basic mental health or substance use disorder recovery skills
- Understanding of substance use disorder treatment and recovery, engagement in treatment and recovery
- Knowledge of health care systems, social service systems, and the criminal justice system
- Knowledge of psychiatric and substance use disorder symptoms and their impact on functioning
- Crisis response procedures and de-escalation techniques
- Supervision by a licensed professional (e.g., LISW, LPCC, LMFT, LPC, LSW, etc.)
- Valid/Current Driver's License
- Ability to travel up to 15%
