Job Summary
Complete timely and accurate documentation in the Uniform Case Record, including referral intake packets, correspondences, entitlement documentation, and logbooks. Review all intake information for new referrals, perform initial screening assessments, and make recommendations regarding acceptance or denial. Conduct annual housing office intake assessments for new tenants and develop Individual Service Plans (ISPs), reporting outcomes and implementing changes every six months. Address monthly progress against specific milestones and targets related to housing and service referrals. Work Saturdays and Sundays for 16 hours per week with no on-call requirements. Associate's Degree in Social Work required; Bachelor's preferred.
Required Qualifications
- Associate's Degree in Social Work
- 1–3 years' experience with relevant experience in Human Services or a related field
- Valid Driver's License
- Car insurance
- Registration
- Access to working vehicle
