Job Summary
Case Manager is responsible for individuals in SVRC Vocational and Community Living Support programs, authoring designated plans of service and implementing them; completes intake and orientation, conducts initial interviews, and elicits participant cooperation; provides coaching on work-related topics, supports community-based participation, transports participants as needed, and maintains documentation per agency and CARF standards. Requires communicating with referral contacts/guardians, drafting progress reviews, and participating in team activities; preferred Bachelor’s degree in human services and demonstrated communication skills; includes driving responsibilities for participants and requires a valid Michigan Chauffeur’s License.
Required Qualifications
- Bachelor’s degree in the human services field or related area preferred
- Effective written and oral communication skills
- Proficient in Microsoft Office
- Ability to work with a diverse population
- Criminal History Background Check and Drug Screen required
- Safe driving record (for drivers)
- License: able to secure a valid State of MI Chauffeur’s License
