Job Summary
Develop and implement treatment plan goals in collaboration with the multidisciplinary team, completing weekly individual Case Management sessions and documenting in DAP format. Facilitate or co-facilitate assigned groups, support new Members through admission and acclimation, and assist with transporting Members to appointments and community service navigation. Provide direct oversight for Members, including daily coaching of living activities, medication observation, meal planning, and crisis de-escalation. Carry out phone coaching and crisis management calls, respond to emergencies, and perform residential roving to ensure safety. Complete incident reports, family contacts, and annual consent reviews within 24 hours of engagement.
Required Qualifications
- High School Diploma
- 3 years' experience in the Mental Health, Behavioral Health or Drug and Alcohol field
- Associates degree
- 2 or more years' experience in the Mental Health, Behavioral Health or Drug and Alcohol Field
- High School Diploma/Associates degree and 1 year experience at Project Transition/ PCS Mental Health
- Valid, non-commercial driver's license
- Clean driving record
- Ability to operate company vehicles in accordance with safety regulations
