Job Summary
Case Manager at Empire Hotel Men's Shelter supporting 175 individuals with substance use histories. Responsibilities include conducting intake and psychosocial assessments, developing and monitoring Individual Living Plans, counseling clients on goals, housing, sobriety, and employment opportunities, liaising with service providers, organizing group activities, maintaining compliant documentation, and providing escort to appointments. Requires experience with homeless/adult populations and familiarity with case management software; bilingual or multilingual skills a plus.
Required Qualifications
- High School/GED Diploma and experience as described below OR Associate's Degree
- One to two years (1-2) year of experience working with homeless adults, substance use and/or mental health populations.
- Knowledge of case management software as well as Microsoft Office Suite
- Bilingual or multilingual a plus
