Job Summary
Conduct outreach, assess individual needs, and determine eligibility for services for women, children, and families affected by substance use disorders. Develop treatment plans, monitor progress, and provide case management, crisis intervention, and supportive counseling using a trauma-informed approach. Coordinate behavioral health, medical, housing, and community services while conducting on-site drug screenings. Complete documentation, participate in review meetings, and advocate for individuals by collaborating with providers and community partners. This role supports long-term wellness for clients from birth through age seven within the Hampton-Newport News Community Services Board's Project Link program.
Required Qualifications
- Bachelor's degree in Human Services (Social Work, Psychology, Sociology or Counseling)
- One (1) year experience providing case management services to individuals with substance use disorders or co-occurring mental health and substance use disorder
- Valid Virginia Driver's License
- Must successfully pass a criminal history fingerprint background investigation
- Must successfully pass a DMV record check
- Must successfully pass a Child Registry search
- Must successfully pass a drug screening test
- Must successfully pass employment reference checks
