Job Summary
Coordinate interdisciplinary care for designated patient populations by reviewing inpatient admission records to ensure proper resource utilization and appropriate level of care determination. Assess discharge needs and facilitate discharge plans while collaborating with OT, PT, RN, RRT, SLP, and LSW teams to achieve financial and quality outcomes. Maintain and enhance payor relationships to support desired results. This PRN weekend role requires licensure in the state of residence, 3–5 years of inpatient experience, and current BLS certification. Required Qualifications
- Current BLS certification
- Three (3) to five (5) years of inpatient experience
- Licensure in the state where the hospital resides is preferred
Desired Qualifications
- Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker
- inpatient experience, preferably in an acute, IRF, or LTACH setting
