Case Manager PRN

Case Manager in a rehabilitation hospital coordinating treatment plans with physicians to ensure patient progression through the continuum of care, monitoring clinical and financial coordination of treatment plans, and managing CMS-length-of-stay within guidelines for patients with spinal cord injury, brain injury, cerebrovascular accident, amputation, neurologic disorders, orthopedic conditions, and arthritis. Requires 3+ years healthcare/clinical experience in a hospital setting and licensure in RN, SW, PT, OT, or SLP.
