Job Summary
Coordinate health care decisions using a systematic approach to improve treatment plans and outcomes across the continuum. Manage care coordination, promote cost-effective care within allotted timeframes, and ensure payments meet patient-related utilization management criteria. Assess psychosocial needs and provide interventions as part of the discharge planning process while implementing safe and appropriate discharge plans. Handle care planning, fiscal management, and payer/referral management to support efficient hospital operations.
Required Qualifications
- Current Licensure per state guidelines in clinical or related discipline
- Bachelor's or Master's in a health or human services discipline
Desired Qualifications
- Previous experience in Case Management and Discharge Planning
- CCM Certification
