Job Summary
Provide fill-in coverage for care coordination services, including psychosocial assessments, discharge planning, crisis support, and community resource referrals. Communicate with patients, families, physicians, and hospital staff to ensure operational continuity during manager absences. Support staff and maintain regulatory compliance while managing patient care coordination. Requires a Master of Social Work or RN license issued by the State of Kansas. Available for as-needed shifts with full-time responsibility readiness.
Required Qualifications
- Master of Social Work
- RN license issued by the State of Kansas
- ability to provide documentation demonstrating employment eligibility as required by the Immigration Reform and Control Act of 1986
