Job Summary
Case Manager collaborates with the physician to provide individualized program management for rehabilitation patients, monitoring clinical and financial coordination of treatment plans to ensure timely, cost-effective, and outcome-driven care. Manages length of stay within CMS guidelines, coordinates community resources including vocational rehabilitation, works with patients with disabilities such as spinal cord injury, brain injury, stroke, amputation, neurologic disorders, and orthopedic conditions, and supports a total rewards package including health and retirement benefits. Requires active licensure as RN or SW; strong communication, negotiation, conflict resolution, organizational, time management, and analytical skills are essential.
Required Qualifications
- Current state licensure as a Registered Nurse
- Licensed Social Worker
- MSW without licensure will be considered
- Minimum of 3 years healthcare and clinical experience in a hospital setting
