Job Summary
Assess, plan, implement, coordinate, monitor, and evaluate options and services to meet patients' health care needs across all phases of wellness and sickness. Collaborate with patients, families, payers, and the multidisciplinary healthcare team to maximize efficient resource utilization and promote quality cost-effective outcomes. Maintain confidentiality, exercise sound judgment, and utilize strong organizational skills while identifying cost and quality issues related to patient care. Requires current Texas licensure as an LVN, RN, or LMSW with three to five years of experience; CCM, CPUR, or ACP certification is preferred. Valid driver's license and reliable transportation are necessary for driving between campuses. Required Qualifications
- Current licensure to practice as licensed vocational nurse, a registered nurse or masters prepared social worker in the State of Texas
- Three to five years' experience in nursing or medical social work
- Prior experience in case management and/or discharge planning
- LVN, RN, or LMSW with CCM, CPUR, or ACP
- Valid drivers' license
- Reliable transportation
- Self-directed
