Job Summary
Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient progress towards outcomes by monitoring care through assessments and records, facilitates discharge planning, conducts utilization reviews, and documents medical necessity and post-discharge needs. Requires adherence to accreditation standards (TJC and CMS) and to safety, infection prevention, and confidentiality. Demonstrates strong communication, teamwork, and time-management skills to coordinate care across internal stakeholders and payors; supports length of stay management and safety.
Required Qualifications
- Relevant Bachelor’s degree; OR Current state clinical licensure; OR 4 years relevant experience required
