Job Summary
Case Manager to join an interdisciplinary clinical team serving individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment at Priest Dr. Recovery Health Center in Tempe, AZ. Full-time position (40 hours/week) with benefits. Responsibilities include assisting clients with identifying treatment and recovery goals, completing assessments, crisis plans, individualized service plans, coordinating care with family and providers, conducting outreach and follow-up (including crisis interventions, missed appointments, and hospital discharges), and pursuing outcomes related to housing, employment, and community tenure. Requires driving (AZ license), background check, TB test, and typing proficiency; training provided and new grads welcome. Educational options include GED/high school diploma with behavioral health experience, an associate, bachelor’s, or master’s degree in behavioral health or related field. Position located at 1642 S. Priest Dr. Bldg. 6 Suite 101, Tempe, AZ 85281.
Required Qualifications
- Bachelor's degree in behavioral health or related field OR equivalent experience (as listed in the posting)
- Valid AZ Driver License
- Valid AZ Fingerprint Clearance card (Level 1) or ability to obtain
- Typing test with 25 wpm or higher
- Background check and pre-employment screenings
- DSM-V knowledge preferred
- Certified Case Manager strongly preferred
