Job Summary
CASE MANAGER responsible for managing a caseload of chronically homeless adults with co-occurring disorders, delivering person-centered intensive case management, developing quarterly treatment/service plans, coordinating with medical/mental health providers, advocating for benefits and legal services, providing crisis intervention, and maintaining timely documentation. The role emphasizes collaboration with interdisciplinary teams, community partnerships, and adherence to ethics and confidentiality within a non-profit social services setting in Brooklyn, NY. Strong emphasis on building client independence, hands-on client-facing support, and ensuring comprehensive services through coordination and advocacy.
Required Qualifications
- Bachelor’s degree in social work, Psychology, or Mental Health (MSW preferred)
- Will consider 5 years or more of Case Management experience
- Residential setting experience preferred
- Strong written and verbal communication skills required
- Bi-lingual (Spanish) preferred
- Working knowledge of computer programs (including email, Microsoft Word and PowerPoint)
