Case Manager - Platform to Employment Youth

Job Summary
Provide trauma-informed, individualized support to disconnected and at-risk youth aged 18-26 through one-on-one case management, mentorship, and wraparound services addressing barriers like housing instability and justice involvement. Develop mutually agreed upon Employment and Service Plans, monitor participant progress, and initiate referrals to agencies for job placement or post-secondary acceptance. Maintain comprehensive case files, participant databases, and internal/external reports while traveling to Norwalk, New Haven, Waterbury, and Hartford to engage with clients and community partners.
Required Qualifications
- Bachelor's degree or equivalent from a four-year college in Human Services, Education, or Business Management
- three years of related experience and/or training
- equivalent combination of education and experience
- Excellent intrapersonal skills with the ability to build rapport, motivate, and maintain positive relationships with youth and colleagues
- Highly organized with the ability to manage multiple cases, meet deadlines, and adapt to changing priorities
- Proficiency in Microsoft Excel for tracking participant progress, data entry, and reporting
- Comfortable working independently and in a team across multiple sites
- Commitment to assisting at-risk and disconnected youth in overcoming barriers to achievement and success in their personal and work lives
- Excellent verbal, presentation, and written communication skills
- Demonstrates strong cultural competence and the ability to work effectively with individuals from diverse backgrounds, fostering an inclusive and respectful workplace
- regular travel to Norwalk, New Haven, Waterbury, and Hartford
Desired Qualifications
- The ability to read, speak and/or interpret Spanish
- training in Professional Accredited Resume Writer (PARW)
