Job Summary
Case Manager provides support services and resource coordination for youth, adults, elders, and families in need of social services assistance. This role connects participants with housing resources, rehabilitation programs, healthcare services, and community support programs; conducts intake interviews, completes comprehensive assessments, develops individualized case plans with measurable goals, and conducts ongoing assessments with quarterly reviews. Responsibilities include coordinating with outside agencies and healthcare, legal, behavioral health, and housing providers; maintaining case notes and records; ensuring confidentiality and compliance; and promoting housing stability through trauma-informed, culturally respectful practices. Requires organizing and supporting participants in obtaining identification and essential documents, assisting with income resources, and participating in community outreach and collaborative efforts. Minimum qualifications include a High School Diploma or GED and two years of related experience; preferred Bachelor’s degree in Social Work or related field and experience in trauma-informed or housing-first environments. This position is classified as Non-Exempt, Full-Time with benefits, located in Oakhurst, CA, and reports to the Social Services Director. Required Qualifications
