Job Summary
Coordinate case management services for youth in care by developing and updating individualized treatment plans and initiating early discharge planning. Build relationships with families, schools, and community partners while assisting with educational planning and transition services. Transport youth to medical, dental, mental health, court, and other appointments, managing follow-up communications with providers. Maintain accurate documentation ensuring compliance with agency and DFPS requirements, and participate in treatment team meetings. Support youth using trauma-informed practices during daily interactions and crisis situations. This role offers a set Monday through Friday schedule with some Saturday shifts at the Phoenix Residential facility in Ellis County.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field (or qualifying equivalent experience)
- A minimum of 1 to 3 years of related work experience
- Minimum age of 23 years
- Valid Texas driver's license with a safe driving record
- Ability to pass required background checks
- Valid Texas driver's license
