Job Summary
Manage cases for assigned clients to ensure services align with Individualized Treatment Plans, utilizing clinical interventions for sobriety, mental health stability, and overall wellbeing. Acquire and maintain contact information for community resources addressing housing, food, medical care, and safety needs. Complete progress notes and documentation within 24 to 48 hours while reviewing case files for audit readiness. Collaborate with referral sources, agencies, and legal entities to coordinate care and facilitate referrals. Achieve individual productivity goals and perform crisis de-escalation and emergency intervention techniques across multiple settings. Work 4-5 days per week from 3:30 to 7:00 PM, maintaining a valid driver's license and using a personal vehicle for duties.
Required Qualifications
- High School Diploma or equivalent
- 3+ years of experience working in the social work field
- Must be certified within 90 days of starting the position on First Aid, CPR, and De-escalation Training
- Be at least 23 years of age
- Have valid driver's license
- Have an acceptable driving record
- Maintain a current and valid Ohio or Michigan driver's license
- Have a good driving record
- Have adequate insurance
- Be willing to use personal car to perform job duties
- Computer skills including Excel capabilities with ability to create reports
