Job Summary
Provide client-centered case management services to individuals experiencing chronic homelessness, leveraging their strengths to achieve self-defined goals. Assist clients with daily life skills, including grocery shopping, budgeting, and navigating public transportation, while conducting regular assessments of mental and physical wellness. Manage crisis situations using de-escalation techniques and collaborate with healthcare partners to coordinate wraparound supports and referrals. Navigate housing stabilization by guiding clients through lease signings, utility setup, and move-in processes, ensuring long-term housing stability. Discern approved financial assistance and help clients obtain necessary documents for employment and benefits, including SSI/SSDI applications. Maintain accurate case records, perform heavy data entry, and comply with all funding and licensure requirements. Transport clients to appointments and agencies as needed.
Required Qualifications
- Bachelor's Degree in social or behavior sciences or administrative field
- minimum 2 years related experience
- Equivalent combination of education and experience
- Proficient in HMIS
- Proficient in Microsoft Office
- Proficient in Outlook
- Proficient in agency database
