Job Summary
Maintain a caseload of 17–30 individuals by conducting weekly housing assessments, developing individualized housing plans, and implementing harm reduction and eviction prevention strategies. Provide residential services including life skills workshops, community-building events, and coordination of job readiness training while visiting apartments to ensure proper maintenance. Engage in crisis intervention, trauma-informed care, and outreach to connect residents with community resources for employment, education, and mentoring. Participate in the emergency on-call schedule, travel to various locations for supportive services, and maintain accurate documentation of all client files and case notes.
Required Qualifications
- Valid California driver's license
- Proof of insurance
- A reliable vehicle
- Ability to lift up to 50 pounds
- Ability to sit for prolonged periods while working at a desk and using a computer
Desired Qualifications
- Bachelor's degree in Social Work or a related field
- Minimum of two (2) years of experience working in chemical health, mental health, recovery, or co-occurring disorders
- At least two (2) years of direct life experience working with long-term homeless, low-income, and diverse populations
- A working knowledge of mental health
