Job Summary
Case Manager at Path of Life Ministries responsible for working with shelter guests and POLM clients to assess assets and needs, develop and monitor Individual Action Plans, track progress, and connect clients to employment, permanent housing, medical/mental health services, benefits, and life-skills resources. Acts as advocate, encourager, and resource to help clients overcome barriers, maintain self-sufficiency, and navigate program requirements; maintain client files, verify intake information, perform client assessments, and provide weekly progress reports and referrals. Includes collaboration with program staff and participation in meetings; requires strong interpersonal and written communication skills and the ability to coordinate multiple tasks in a high-pressure environment.
Required Qualifications
- Associate's degree in Sociology, Psychology, Social Work, or related field (Required)
- Bachelor’s degree in Sociology, Psychology, Social Work, or related field (Preferred)
- Two years of experience in case management services (Preferred)
- Experience working with homeless and mentally ill populations (Preferred)
- Employment eligibility verification required
- Successful completion of background screening required
- Peer Support and/or WRAP Certified preferred
- Motivational Interviewing, Trauma Informed Care, and cognitive approaches to social services (knowledge)
- Knowledge of social class dynamics and poverty in America
- Proficiency in Microsoft Office, Excel, and (preferably) Access
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Maintain confidentiality
- Highly motivated with ability to manage multiple tasks
- Ability to work with diverse communities and exercise mature judgment
- Problem-solving and conflict resolution skills
- Physical ability considerations like sitting/standing balance
