Job Summary
Act as a liaison between therapists, care team members, clients, and families to coordinate the best care and arrange family visits. Assess, plan, implement, and monitor client care while advocating for resources such as housing, employment, and social services. Attend court hearings, facilitate individual and group meetings, and address client concerns to support recovery and independence. Ensure documentation and billing adhere to policy and EHR standards, maintain compliance with HIPAA and audit criteria, and present accurate case management reports. Oversee the Individual Plan of Service, assist in crisis intervention and discharge planning, and identify gaps in service utilization. Maintain an organized schedule, reduce client rescheduling, and assume additional duties as assigned by leadership.
Required Qualifications
- High School Diploma or GED
- Minimum of 3 years of case management experience
- Ability to work under stressful conditions
- Ability to work irregular hours
- Excellent verbal and written communication skills
- Strong grammar and writing skills
- Ability to interact and assist in a friendly, compassionate, and professional manner with colleagues, providers, clients, and their families
- Competency in the Electronic Health Record (EHR) program
- Competency in telemedicine services
- Competency in any additional software programs utilized by the company
- Fine dexterity
- Adept handling
- Proficient gripping abilities
- Pass all federal and state background checks
- Clean Motor Vehicle Record
- Educational and work history confirmed in compliance with company policies
