Job Summary
Coordinate interdisciplinary care for a designated patient population by reviewing inpatient admission records to ensure proper resource utilization and appropriate level of care determination. Assess and identify discharge needs to facilitate coordinated discharge plans while working collaboratively with the OT, PT, RN, RRT, SLP, and LSW teams to achieve desired financial and quality outcomes. Maintain and enhance payor relationships to support overall care delivery. Requires three to five years of inpatient experience, preferably in an acute, IRF, or LTACH setting, with current BLS certification.
Required Qualifications
- Current BLS certification
- Three (3) to five (5) years of inpatient experience
- Licensure in the state where the hospital resides is preferred
Desired Qualifications
- Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker
- inpatient experience, preferably in an acute, IRF, or LTACH setting
