Case Manager - Part Time 24-40 Hours

Meet with every patient and family within one working day of admission to begin discharge planning assessments. Attend interdisciplinary team meetings to establish treatment plans with realistic goals and target dates. Collaborate with the healthcare team to coordinate timely transfers to the next level of care and manage referrals. Maintain patient privacy and confidentiality while documenting all activities in the medical record. Act as utilization review coordinators to monitor care levels and obtain payor approvals for continued treatment. Notify the team and business office of any exclusions or level of care changes. This role supports Whittier Rehabilitation Hospital's mission to help medically complex patients make progress over their 2-3 week stays.
