Job Summary
Conduct comprehensive eligibility screenings and intake assessments to determine Veteran eligibility for program services. Deliver presentations to community organizations and coordinate with stakeholders during case conferencing meetings. Develop and maintain a comprehensive database of community resources while advocating for access to housing, utility, and medical assistance. Assist Veterans in securing safe, affordable housing and provide ongoing stabilization support to prevent homelessness. Maintain accurate case notes and client files in accordance with agency policies. Collaborate with supervisory staff to review outreach strategies and prepare reports for leadership. Work nights, weekends, and holidays as required, utilizing a personal vehicle for daily outreach and travel within assigned counties.
Required Qualifications
- Bachelor's Degree in Social Work, Sociology, Psychology or related field
- 1+ years case management experience
- Driver's License with clear record
- Must maintain regular and acceptable attendance
- Must have daily use of a vehicle
- Must be available and willing to travel to various locations
- Must be available and willing to work nights, weekends and holidays
- Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others
Desired Qualifications
- 3+ years case management experience
- 2+ years in a customer service focused environment
- Experience with homeless, low income, veterans & their families
- LMSW, LBSW, LMFT
