Job Summary
Conduct comprehensive eligibility screenings and intake assessments for Veterans experiencing or at risk of homelessness. Deliver client-centered case management by developing individualized service plans, coordinating housing assistance, utility support, and referrals to mental health and medical resources. Perform daily outreach across shelters, encampments, and community events to connect participants with supportive services and promote program awareness. Maintain accurate case files, document service progress, and prepare reports for leadership while collaborating with supervisors on outreach strategies and program evaluation. Assist in securing and maintaining safe, affordable housing through direct landlord relations and stabilization support.
Required Qualifications
- Bachelor's Degree in Social Work, Sociology, Psychology or related field
- 1+ years case management experience
- Driver's License with clear record
- Must maintain regular and acceptable attendance
- Must have daily use of a vehicle
- Must be available and willing to travel to various locations
- Must be available and willing to work nights, weekends and holidays
- Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others
Desired Qualifications
- 3+ years case management experience
- 2+ years in a customer service focused environment
- Experience with homeless, low income, veterans & their families
- LMSW, LBSW, LMFT
