Job Summary
Conduct comprehensive behavioral health assessments and develop individualized re-entry care plans for participants experiencing substance use and mental health conditions. Coordinate access to medical, housing, employment, and community services while facilitating referrals across correctional facilities, courts, and managed care organizations. Maintain accurate documentation of all services and assessments in compliance with Washington State Medicaid Re-Entry Demonstration Initiative and federal confidentiality regulations. Collaborate with multidisciplinary treatment teams and external stakeholders to support participant recovery and successful community integration.
Required Qualifications
- Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Counseling, or a closely related field preferred
- Washington State Department of Health credential required or ability to obtain and maintain required credentialing
- Minimum of one year of paid or volunteer experience working with individuals experiencing mental health, substance use, co-occurring disorders, or related behavioral health conditions
- Working knowledge of behavioral health recovery principles, trauma-informed care, and participant-centered service delivery
- Familiarity with applicable Washington State behavioral health regulations, including RCW 71.05 and WAC 246-341
Desired Qualifications
- Substance Use Disorder Professional Trainee (SUDPT) credential
- Experience providing re-entry, care coordination, case management, or community behavioral health services
