Case Manager - On Ramp

Job Summary
Case Manager for the On Ramp program provides direct client services and manages a rapid re-housing caseload for pregnant and parenting young adults, performing field and office duties, documenting activities, and implementing the TIP Model of case management. Responsibilities include delivering supportive services to promote self-sufficiency and community integration, coordinating with housing resources, and maintaining caseloads across office and community sites. Requires a bachelor’s degree in social work or related field and 2 years of relevant experience, with knowledge of Housing First, strong organizational and communication skills, attention to detail, and familiarity with community resources and tenant rights. English fluency is required; Spanish is a plus. Proficiency in Microsoft Office is expected.
Required Qualifications
- Minimum of a bachelor’s degree in social work or a related human service field
- 2 years’ experience working with transition age youth and/or individuals affected by homelessness, mental health or substance abuse
- Requires general knowledge on “Housing First” philosophy and strategies
- Possess strong organizational skills with ability to meet a demanding workload
- Detail-oriented to complete requirements of files and maintain contract compliance
- Excellent communication skills, particularly listening, mediation, and writing skills
- Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths-based” case management a plus
- Demonstrated knowledge of community resources, social service agencies, and landlords
- Fluency in the English language is required
- Experience with computer and knowledge of Microsoft Office
