Job Summary
In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team, provide Intensive Case Management Services (ICMS) to clients recently released from jail who are experiencing homelessness. Coordinate client release and assist in attaining permanent supportive housing through field-based case management for a diverse group facing medical, mental health, and substance use barriers. Maintain weekly contact, document in County-approved databases, connect clients to medical/mental health/substance use resources, help obtain entitled benefits, participate in court hearings, maintain up-to-date biopsychosocial assessments, develop and review goal plans to improve housing stability, manage client files and data, ensure HIPAA compliance, coordinate transportation and other supports, transport clients to appointments across the greater Los Angeles area, build trusting relationships to promote independence and recovery, and collaborate with staff to deliver wrap-around services in both field and office settings.
Required Qualifications
- Bachelor’s degree preferred; or a minimum two years’ case management experience working with individuals experiencing homelessness, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience
- Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record
- Able to pass a background check for jail clearance
- Able to transport clients in your vehicle
- Demonstrated knowledge of case management
- Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
- Skill in non-violent crisis intervention
- Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs
- Detail oriented with strong time management, organizational, written, verbal, interpersonal, and computer skills
- Able to obtain and maintain CPR/First Aid certification
