Job Summary
Work with a diverse caseload of 15 justice-involved clients recently released from jail to provide intensive case management, linking them to medical, mental health, and substance abuse resources. Collaborate with clients to develop goal plans, obtain benefits, and coordinate appointments, including transporting clients to healthcare providers and community agencies in the greater Los Angeles area. Maintain weekly contact, document all interactions in approved databases, and ensure up-to-date biopsychosocial assessments while adhering to the "Whatever It Takes" model. Participate in court hearings, staff meetings, and trainings to support housing stability and recovery for clients experiencing complex trauma.
Required Qualifications
- High School Diploma or GED or Equivalent
- Valid CA Driver's license
- reliable car

- Able to transport clients in your vehicle
- Able to pass a background check for jail clearance
- Demonstrated knowledge of case management
- Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
- Skill in non-violent crisis intervention
- Computer literate
- able to effectively use computerized database for client file management

Desired Qualifications
- Bachelor's degree preferred; or a minimum two (2) years' case management experience working with individuals experiencing homelessness, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience
