Job Summary

Conduct intensive case management for a diverse caseload of 20 justice-involved clients transitioning from homelessness to permanent supportive housing. Maintain weekly contact with all clients, document interactions in approved databases, and ensure services reflect the "Whatever It Takes" trauma-informed model. Link clients to medical, mental health, and vocational resources while assisting with benefit enrollment and court progress reporting. Transport clients to healthcare providers and community agencies using a reliable vehicle, and develop comprehensive goal plans to improve housing stability and independence. Collaborate with the staff team in weekly meetings and maintain accurate, confidential client files per HIPAA guidelines.

Required Qualifications

  • Valid CA Driver's license

  • Able to pass a background check for jail clearance
  • Demonstrated knowledge of case management
  • Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers

  • able to effectively use computerized database for client file management

Desired Qualifications

  • Bachelor's degree preferred
  • minimum two (2) years' case management experience working with individuals experiencing homelessness
  • preferably those living with mental illness and/or substance addictions
  • an equivalent combination of education and experience
  • Minimum 30 WPM typing speed
  • Bachelor's degree
