Job Summary
Case Manager – Housing Support Services: Provide brokerage of community resources, conduct one-on-one case management, ongoing assessment of clients’ progress and needs, advocacy, and implementation of group activities for formerly homeless, disabled individuals. Core responsibilities include developing and updating treatment/service plans, linking residents to services to prevent homelessness, assisting with housing stability, coordinating with property management and clinical teams, and ensuring documentation and data entry are completed in a timely manner. The role requires collaboration with a multi-disciplinary team, strong customer service, knowledge of local homeless services, and the ability to drive and liaison with partners. Work location is Denver, CO at 3999 Colorado Blvd. Credentialing privileges are required prior to start. The position emphasizes trauma-informed, client-centered support to help residents maintain housing and access benefits.
Required Qualifications
- Bachelor’s degree in social service or related field preferred
- Experience and interest working with disenfranchised persons
- Bilingual preferred but not required
- Valid driver’s license, ability to pass automobile insurance carrier’s motor vehicle record investigation and ability to drive a 15-passenger van
- Able to pass automobile insurance carrier’s motor vehicle record investigation and criminal background investigation
- Knowledge of homeless issues and demonstrated sensitivity to underserved populations
- Skills in Microsoft 365 (Word, Excel, PowerPoint, Outlook, Teams)
- Strong interpersonal and communication skills
- Ability to develop treatment/service plans and linkages to address resident needs
- Ability to advocate for individuals in housing stability
- Ability to manage multiple responsibilities and ensure data accuracy
- Experience with documentation in electronic health records or equivalent systems
