Job Summary
Provide community-based case management and advocacy for at-risk youth and young adult households facing homelessness. Responsibilities include outreach to local programs, assessing needs and risks for housing stability, coordinating financial assistance, helping participants develop realistic housing plans with measurable steps, maintaining ongoing contact for crisis intervention and life skills development, documenting progress and updating service plans, maintaining organized participant files, collecting HMIS data, ensuring habitable housing, participating in team meetings, and applying housing-first/trauma-informed care approaches while complying with COA and regulatory requirements. Requires strong organizational, interpersonal, and communication skills, willingness to learn best-practice models, and a valid Ohio driver’s license.
Required Qualifications
- High school diploma or equivalent
- Valid Ohio Driver’s License
- ability to meet driving insurability requirements
- criminal background check (Ohio Bureau of Criminal Investigation)
- experience with homelessness, at risk youth, domestic violence, chemical dependency, and/or mental health preferred
