Job Summary
Case Manager responsible for developing and implementing individualized service plans for clients in Louisiana, coordinating with clients, families, providers, and state agencies, and ensuring compliance with Medicaid HCBS programs. Requires understanding of ADLs/IDLs, ability to coordinate between client homes and community resources, and proficiency with documentation and case management software. Must hold a valid driver’s license and be able to travel between client locations within Lafayette, LA.
Required Qualifications
- High school diploma or GED
- Associate’s or Bachelor’s degree preferred in Human Services, Social Work, Psychology, or related field
- 1–2 years experience with developmental disabilities, elderly, or vulnerable populations
- Familiarity with Louisiana Medicaid Waiver Programs (CCW, ROW, SIL, HCBS)
- Knowledge of ADLs/IDLS
- Ability to create, review, and implement ISPs
- Basic knowledge of medication administration guidelines for Direct Service Workers
- Strong organizational, documentation, and time management skills
- Proficiency in Microsoft Office, case management software, and EHR systems
- Excellent written and verbal communication with clients, families, providers, and state agencies
- Conflict resolution and problem-solving skills
- Current valid driver’s license
- Reliable transportation with proof of insurance
- Ability to pass state-required background checks and drug screening
- Completion of state-required trainings (Abuse/Neglect Prevention, HIPAA, Person-Centered Planning, Crisis Intervention)
- CPR and First Aid certification preferred
- Physical ability to sit/stand/walk for extended periods and lift up to 25 pounds as needed
