Job Summary
The Case Manager at EAC Network is responsible for providing trauma-informed case management and direct services for minor victims of sexual abuse and their non-offending family members. Key responsibilities include conducting intake assessments, developing individualized service plans, serving as a primary point of contact for families, facilitating referrals to community services, offering crisis intervention, documenting client services in databases, and coordinating care through multidisciplinary team meetings. This role requires a Bachelor's Degree and at least two years of relevant experience, along with proficiency in computer applications. Additional preferred qualifications include bilingual fluency in English and Spanish.
Required Qualifications
- Bachelor's Degree in Mental Health Counseling, Social Work, Forensic Psychology, Behavioral Health or related field preferred
- Minimum of two years of relevant experience
- Must have access to automobile and possess valid NYS driver's license
- Excellent communication, organizational and interpersonal skills
