Job Summary
Lead complex discharge planning by collaborating with physicians and RN Case Managers to assess, plan, implement, and evaluate proactive, safe discharge strategies for psychosocially complex cases. Facilitate care coordination, maintain current knowledge of community resources and post-acute options, and ensure accurate communication of outpatient services, hospitalization details, and treatment plans. Provide guidance on medically complex cases while adhering to UVA Health Northern VA Culpeper policies and maintaining patient confidentiality. This full-time role includes rotating weekends and holidays, on-call responsibilities, and potential travel between campus buildings and remote facilities. Candidates must possess a Master's Degree in Social Work and 2+ years of professional social work or healthcare experience.
Required Qualifications
- Master's Degree in Social Work from school accredited by the Council on Social Work Education
- Weekend and holiday coverage
- Ability to lift/push/pull 100 lbs
- Proficient communicative, auditory, and visual skills
- Attention to detail and ability to write legibly
- May require the use of safety equipment, such as HEPA mask, for infection prevention
- On call responsibilities as directed
- Ability to travel between campus buildings, remote facilities, and out of town as needed
Desired Qualifications
- 2 or more years of professional social work experience
- 2 or more years of healthcare experience
- Case Management or related experience
- Knowledge/Competence of Microsoft Office products and keyboarding
