Job Summary
Case Manager providing services to individuals with severe and persistent mental illness to maintain stability and reduce psychiatric hospitalization. Responsibilities include intake/referral, assessments, service planning, crisis intervention, advocacy, linking to medical and clinical services, determining level of care, developing and monitoring service plans, coordinating transfers/discharges, maintaining documentation, ensuring rights and compliance, arranging transportation, and supporting culturally competent, recovery-based, trauma-informed practices. Requires at least a bachelor’s degree in social work or a related field (or equivalent), 1+ year mental health experience, and relevant licensure eligibility in Michigan.
Required Qualifications
- Minimum one-year experience in the mental health field with target population
- Bachelor's degree in social work or human services field
- Current licensure permitted by the State of Michigan (LBSW/LMSW/LLPC/LPC or similar) preferred
- Ability to perform intake, assessments, service planning, crisis intervention, and advocacy
- Ability to coordinate transportation and access to clinical services
- Criminal records screening and driving record checks required
- Valid Michigan driver's license with a good driving record and access to insured vehicle
- Familiarity with Microsoft Office; strong verbal and written communication
- Knowledge of culturally competent, recovery-based practices and trauma-informed care
- Must be able to meet intervention certification requirements (MANDT)
