Job Summary
Develop continuing care plans and resources for clients prior to discharge while managing a caseload of up to 15 individuals. Collaborate with clinicians, family wellness teams, and referral sources to create clinically appropriate aftercare strategies and provide weekly status updates. Market facility programs by answering inquiries and relay payment and insurance information necessary for admission. Facilitate weekly group sessions educating new residential clients on the continuing care process and appropriate levels of care. Maintain accurate documentation in CRM and EMR systems while complying with federal, state, and accreditation regulatory requirements. Complete all daily scheduled tasks and provide information to family members and providers regarding client commitment to aftercare planning.
Required Qualifications
- Minimum High School education
- 2 Years of experience in human services, addiction treatment or related field
- Must have basic computer skills including competence in software applications such as Microsoft Office suite, Adobe and use of internet applications and in using and/or connecting wireless computer hardware, the ability to project to remote devices, etc.
- Ability to multitask/prioritize specific caseload needs
- Ability to adapt to a changing environment
- Ability to work both independently and collaboratively as necessary
- Strong communication skills
- Excellent organization and time management skills
- Fully vaccinated for Covid-19 (including regularly scheduled boosters) and the Flu as recommended by the CDC
Desired Qualifications
- Bachelor's Degree (B.S., B.A., or B.S.S.W.) from a four-year accredited college or university
