Job Summary
Screen and assess elderly candidates for the Homebound Meal Program, then visit homes to determine client needs and develop service plans. Distribute educational materials, administer nutritional screenings, and refer clients to nutrition counseling or other community agencies as required. Complete necessary paperwork, maintain records in SERV Tracker software, and prepare reports for federal, state, and local programs. Work Monday through Friday from 9:00 am to 3:00 pm with paid vacation, sick time, and supplemental insurance. Maintain confidentiality, manage difficult situations, and contribute to fundraising efforts while adhering to all policies and procedures.
Required Qualifications
- high school diploma
- one to two years of related experience and/or training
- Valid driver's license
- Microsoft Word software
- Microsoft Excel spreadsheet software
- Ability to learn specific software needed for the job, i.e., SERV Tracker
- Must be able to lift and/or move up to 10 pounds
