Job Summary
Provide case management within an interdisciplinary clinical team for individuals with a Serious Mental Illness or mental health diagnosis under Court Ordered Treatment at the Mitchell Clinic. Responsibilities include completing assessments, crisis plans, and individualized service plans; coordinating care with members, families, and providers; conducting outreach and follow-up related to crises, missed appointments, and hospital discharges; pursuing best-practice outcomes in housing, employment, and community tenure; and supporting ongoing professional development. Requires driving in Arizona, proficiency with electronic medical records (NextGen), and strong communication. Required Qualifications
- GED or High School Diploma with 1.5 years of behavioral health experience
- Associates degree in medical or behavioral health
- Bachelor's degree in behavioral health or related field
- Master's degree or higher in behavioral health
- Valid AZ Driver License
- Arizona Fingerprint Clearance (Level 1)
- Certified Case Manager strongly preferred
- Pass background check, TB test and other pre-employment screening
