Job Summary
Case Manager roles focus on integrated care management for adults and behavioral health home services, coordinating assessment, planning, referral, and monitoring to support recovery. Responsibilities include developing and monitoring the Individual Community Support Plan, comprehensive care management and coordination, facilitating Recovery Team Meetings, health promotion and wellness, client and family support, referrals to community services, and where available referral to Peer Support Services. Must have a bachelor’s degree in behavioral sciences or related field and at least 2,000 hours of supervised relevant experience; requires ability to pass MVR check and maintain a valid driver’s license and vehicle insurance. Opportunities include three tiered Integrated Care Manager pathways (full-time/part-time options) with Minneapolis, MN location and salary ranges around $45,760–$47,840 depending on tier. Required Qualifications
- Bachelor’s degree in behavioral sciences or related fields (including social work, psychology, nursing) from an accredited college or university
- At least 2,000 hours of supervised experience delivering services to adults or children with mental illness, substance use disorder, emotional disturbance, traumatic brain injury, or developmental disabilities
- Complete training on mental illness recovery, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects
- Ability to pass MVR check and maintain a valid driver’s license and vehicle insurance
