Case Manager

Job Summary
Case Manager will assist with the execution of South Georgia Healthy Start's daily activities to support women, infants, and families in rural South Georgia. Responsibilities include recruiting, enrolling, assessing, and retaining participants; providing care navigation and social services enrollment assistance; performing routine check-ins; delivering evidence-based curricula and health education; conducting parenting education classes; engaging community partners within the SGHS service region; and may lead committees or initiatives within SGHS. Requires a bachelor's degree and 2+ years relevant experience, with travel and strong interpersonal, verbal and written communication skills; experience with cloud-based case-management systems and a valid driver’s license.
Required Qualifications
- Bachelor's degree from an accredited college/university
- two or more years relevant experience (case management, patient navigation, health education, community engagement, survey research, interventionist work, or doula/birthworker experience)
- Valid driver's license and insurability by university carrier
- Ability to travel regularly within 10-county SGHS project area and to meetings/trainings/events throughout the state and other states
