Job Summary
Case Manager at Mental Health Kokua (MHK) in Hilo, Hawaiʻi Island. Coordinate care and advocate for adults with mental health challenges across behavioral health, medical, housing, and community resources. Provide recovery-oriented, trauma-informed, person-centered support; assist with transitions following hospitalization or major life changes; collaborate with families, landlords, and community providers; respond to crises; complete documentation and ensure compliance; participate in supervision and interdisciplinary team meetings; travel across Hawaiʻi Island for client visits, appointments, and meetings; requires a bachelor’s degree in a behavioral health field (preferred) and relevant case-management experience.
Required Qualifications
- Ability to pass a criminal background check and drug/alcohol screening
- Valid Hawaiʻi driver's license, current vehicle insurance, and safety inspection
- Ability to travel throughout Hawaiʻi Island for client visits, appointments, and meetings
- Ability to lift up to 25 lbs and respond to emergencies as needed
- TB clearance, CPR, and First Aid (or willingness to obtain)
- Demonstrated strong communication, organization, documentation, advocacy, and relationship-building skills
