Job Summary
Conduct needs assessments, link resources, and monitor services for individuals with substance use and/or co-occurring disorders with the MAT Team. Coordinate services and resources for clients to maintain community integration, while ensuring documentation process requirements are met. Provide assistance with transportation as needed. The role includes on-call rotation and requires a Bachelor's degree in Education, Social Work, Psychology, or Human Services with one year of experience in disability, mental health, or substance use settings. QMHP certification is required; CSAC is beneficial. Additional compensation is available for employees who speak multiple languages.
Required Qualifications
- Bachelor's Degree in Education, Social Work, Psychology or Human Services related field
- at least one year experience in a setting working with individuals with disabilities, mental health needs, substance use or Developmental Disabilities
- QMHP is required through the Virginia Board of Counseling
Desired Qualifications
- CSAC is beneficial
- Previous Case Management experience
- speak multiple languages
