Job Summary
The Medical Case Manager (CMC) at SNAHC coordinates referrals and care plans for patients with complex physical, behavioral, dental, and social needs, maintaining engagement through in-person visits, calls, and community outreach. Responsibilities include client interviews, data collection, confidential record-keeping, coordinating with healthcare providers and social services, participating in inter-department care conferences, ensuring updated care plans and documentation, tracking patient panels in the Population Health Management System (i2i), and supporting quality improvement and regulatory compliance. Requires strong communication, organizational, and computer skills, experience with EHR systems, and the ability to work across diverse populations. This role may travel between sites as needed and may float to different locations depending on business needs. Prefer Bachelor's degree and experience in FQHC settings, familiarity with NextGen, and bilingual abilities.
Required Qualifications
- Two years or more in a Health, Social Services, or related field
- Experience working in an Electronic Health Record (EHR) environment
- Experience and knowledge of the culture and environmental issues significant to homeless individuals, low-income families, mental health patients, and individuals involved in the criminal justice system
- Experience with interventions such as motivational interviewing, de-escalation, harm reduction, and trauma-informed care in a diverse clinical setting
- Bachelor’s Degree or equivalent work experience in a Health, Social Services, or related field
