Job Summary
Conduct daily assessments, interviews, and documentation for clients with complex mental health or substance abuse needs. Provide tenancy support to assist clients in finding and maintaining housing stability, while helping them access medical care, social security, Medicaid/Medicare, and other essential resources. Facilitate rehabilitation groups and individual meetings, document interactions to develop person-centered plans, and collaborate with therapists, psychiatrists, and family members. Offer support for daily living activities, crisis intervention, and transportation coordination to community services, including medication monitoring. Work Monday through Friday with on-call availability for emergencies, adhering to ethical standards and completing required training in NCI, CPR/First Aid, and HIPAA.
Required Qualifications
- Bachelor's degree in psychology, counseling, social work, rehabilitation, or behavioral science
- Must be at least 21 years of age
- Valid driver's license with a clean driving record
- Ability to pass a criminal background screening
- Strong oral and written communication skills in English
- Ability to perform job functions with minimal supervision while adhering to the highest ethical standards
- Willingness to complete required training in NCI, CPR/First Aid, HIPAA, and other necessary certifications
- Must be able to lift at least 25 pounds
- Ability to navigate multiple flights of stairs
- Ability to tolerate exposure to various weather conditions
- Ability to travel using public or private transportation as needed
- Ability to sit, stand, kneel, and perform a variety of movements throughout the workday
