Job Summary
Case Manager for Martins Ferry, OH providing community-based supports to adults with severe and persistent mental illness (SPMI). Works within an inter-disciplinary team to develop and implement individualized treatment plans, connect patients to housing, healthcare, and employment resources, advocate for patients’ rights, promote healthy lifestyle choices, and deliver crisis intervention as needed. Duties include resource development, care coordination, monitoring progress, maintaining records, and ensuring goal attainment. Requires AA/BA minimum; LSW/LPC, CCDCII or II strongly preferred. Valid driver’s license and insurance; vaccination for flu is required. Strong interpersonal and communication skills and ability to work collaboratively within an interdisciplinary team are essential.
